- Serum 25-hydroxyvitamin D [25(OH)D] is the preferred marker for assessing vitamin D status.
- Vitamin D levels may be influenced by sunlight exposure, diet, seasonal variation, malabsorption disorders, obesity, and certain medications.
- Results should be interpreted in conjunction with calcium, phosphate, parathyroid hormone (PTH), and the clinical condition when appropriate.
What commonly goes wrong with a Vitamin D, 25-Hydroxy
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- The test for vitamin D status is 25-hydroxy vitamin D. The 1,25-dihydroxy form is a different test with different indications and is ordered by mistake surprisingly often; the two are not interchangeable and cannot be compared against each other's ranges.
- Units are a real source of harm here. ng/mL and nmol/L differ by a factor of about 2.5, and a report that prints a value in one unit beside a reference range in the other manufactures either a false deficiency or a false normal.
- High dose weekly supplement sachets are widely prescribed in India, and a level drawn a few days after a dose reflects that dose rather than the patient's underlying status. Test before supplementation is started, or once the level has stabilised on treatment, which in practice means not repeating earlier than about three months.
- Different platforms and different labs give measurably different 25-OH vitamin D values. Serial monitoring should stay with the same laboratory and method, and a result from one lab should not be read as a change against another lab's earlier figure.
