- Serum ferritin reflects body iron stores and should be interpreted in conjunction with iron studies, complete blood count (CBC), and clinical findings.
- Ferritin is an acute-phase reactant and may be elevated in inflammation, infection, liver disease, malignancy, or chronic illness independent of iron status.
- A low serum ferritin concentration is highly suggestive of iron deficiency in the absence of significant inflammation.
What commonly goes wrong with a Serum Ferritin
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Ferritin is an acute phase reactant. Infection, any inflammatory illness, liver disease and malignancy all raise it, so a normal or high ferritin does not rule out depleted iron stores and it is normally read alongside CRP.
- The test is asymmetric in what it proves. A low ferritin is highly specific for low iron stores, while a raised ferritin is non specific and is not on its own evidence of iron overload.
- ng/mL and micrograms/L are numerically identical for ferritin. Reports that switch units between visits look like a large change when nothing has actually moved, so keep the unit fixed on the format.
- Reference limits differ by assay platform and by sex. Follow a patient on the same lab and the same method, and treat a value from a different lab as a fresh baseline rather than as part of the trend.
