How a Vitamin B12 report is read
- Serum vitamin B12 should be interpreted in conjunction with the patient's clinical findings, complete blood count (CBC), and peripheral smear where appropriate.
- Borderline vitamin B12 levels may require further evaluation with methylmalonic acid (MMA) or homocysteine if clinically indicated.
- Recent vitamin B12 supplementation, liver disease, and myeloproliferative disorders may influence serum vitamin B12 concentrations.
What commonly goes wrong with a Vitamin B12
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- A recent B12 injection or oral supplement makes the level uninterpretable. The serum value then reflects what was given, not the patient's stores, so the date of the last dose belongs on the request form.
- A result in the low-normal range does not exclude deficiency. Serum B12 measures total circulating B12, including the fraction not available to cells, which is why functional markers such as homocysteine or methylmalonic acid are used when the number and the clinical picture disagree.
- B12 is light sensitive. Keep the sample protected from light and separate it promptly rather than leaving it out with the rest of the batch.
- Serum B12 falls physiologically in pregnancy, so a pregnant patient's value read against the ordinary adult range over-calls deficiency.
