How a Iron Profile report is read
- Iron studies should be interpreted in conjunction with serum ferritin, complete blood count (CBC), and clinical findings.
- Serum iron concentrations exhibit diurnal variation and may be influenced by recent dietary iron intake.
- Interpretation of iron studies may be affected by inflammation, infection, pregnancy, liver disease, and recent iron therapy.
What commonly goes wrong with a Iron Profile
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Collect a fasting morning sample. Serum iron swings substantially through the day and from day to day in the same person, so an afternoon value cannot be compared with a morning one and an isolated low reading may mean nothing.
- Recent oral iron pushes serum iron up sharply for several hours. The sample must be taken before the day's dose, or after whatever gap the treating doctor specifies, otherwise the profile is uninterpretable.
- Serum iron alone is not a result. Transferrin saturation is serum iron divided by TIBC and expressed as a percentage, and both figures have to be on the report for the ratio to be checkable, particularly because TIBC itself falls in inflammation and chronic illness.
- Use a plain serum tube. Haemolysis releases iron from red cells and falsely raises the serum iron, and EDTA plasma is unsuitable because the anticoagulant chelates metal ions and interferes with the measurement.
