How a Complete Urine Examination report is read
- Proper collection of a clean-catch midstream urine specimen is essential for accurate interpretation.
- Positive nitrite and leukocyte esterase findings suggest urinary tract infection and should be correlated with urine microscopy and culture where indicated.
- Microscopic findings should be interpreted in conjunction with the chemical examination.
What commonly goes wrong with a Complete Urine Examination
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Urine must be examined as soon as possible and within two hours of being passed, or refrigerated at 2 to 8 degrees Celsius until it is. In a sample left standing, casts and red cells break down, bacteria multiply and the pH drifts upwards, so the microscopy describes a specimen that no longer matches what the patient gave.
- A clean-catch midstream sample is the whole point of the test. Squamous epithelial cells alongside pus cells usually mean contamination during collection rather than urinary tract disease, and the answer is a fresh, properly collected sample rather than a footnote on the report.
- The dipstick and the microscopy answer different questions and both belong on the report. A strip positive for blood with no red cells in the sediment points to free haemoglobin or myoglobin, and the protein pad detects mainly albumin, so light chains are largely missed.
- A negative nitrite does not rule out infection. Many urinary organisms do not reduce nitrate, and the pad needs urine that has sat in the bladder for several hours, which a random daytime sample usually has not.
