How a Liver Function Test (LFT) report is read
- Liver function tests should be interpreted in conjunction with the patient's clinical findings and other laboratory investigations.
- Abnormal liver enzymes may be seen in liver disease as well as non-hepatic conditions. Clinical correlation is advised.
- Serum bilirubin, transaminases, alkaline phosphatase, albumin, and total protein should be interpreted together rather than individually.
What commonly goes wrong with a Liver Function Test (LFT)
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Haemolysis invalidates the AST. Red cells are rich in AST, so even mild haemolysis pushes it up and makes the AST:ALT ratio meaningless; the sample is recollected, not reported with a note.
- A raised ALP is not automatically a liver finding. ALP is physiologically high in growing children and adolescents and in the third trimester of pregnancy, and it also rises in bone disease.
- Globulin and the A:G ratio are calculated, not measured. If total protein or albumin is keyed in wrongly the ratio silently follows, so check that globulin equals total protein minus albumin before the report goes out.
- Bilirubin degrades in light. A tube or separated serum left on the bench or near a window reads falsely low, which quietly changes the whole panel's interpretation.
