How a Serum Bilirubin report is read
- Serum bilirubin should be interpreted together with the liver enzymes and the clinical picture.
- A predominant rise in unconjugated (indirect) bilirubin suggests haemolysis or Gilbert's syndrome, while a conjugated (direct) rise suggests hepatocellular or obstructive causes.
- Reference ranges vary with age (notably in neonates) and method.
What commonly goes wrong with a Serum Bilirubin
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Indirect bilirubin is calculated as total minus direct, never measured. A report where direct exceeds total is arithmetically impossible and points to an analytical or transcription error, so the panel is repeated.
- Bilirubin is light sensitive. Samples left uncovered on the bench lose bilirubin over hours and read falsely low, which matters most in exactly the cases where the number decides the next step.
- In a newborn the value only means something against the baby's age in hours. A total bilirubin issued without the time of birth and the time of collection cannot be plotted by the treating doctor.
- Haemolysis and lipaemia both interfere with the diazo reaction, so a grossly haemolysed or milky sample should be rejected rather than reported with a disclaimer.
