How a Complete Blood Count (CBC) report is read
- The complete blood count should be interpreted in conjunction with the patient's clinical findings and other laboratory investigations.
- A peripheral smear review is advised where counts are markedly abnormal or cell morphology is in question.
- Reference ranges vary with age, sex, altitude and pregnancy, and with the analyser and method used by the laboratory.
What commonly goes wrong with a Complete Blood Count (CBC)
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- The EDTA tube must be filled to the mark and mixed immediately. An underfilled tube leaves excess anticoagulant that shrinks the red cells and lowers the haematocrit and MCV, and a tube that has clotted quietly gives a falsely low platelet count with no obvious warning.
- A low platelet count from the analyser should not be released until a peripheral smear has been checked for clumping. EDTA induced platelet clumping is common and produces a frighteningly low count in a patient who is perfectly well.
- Blood drawn from the arm carrying an IV line is diluted by the fluid and reads as sudden anaemia. Draw from the opposite arm, and note it on the requisition when there is no choice.
- A low MCV is not automatically iron deficiency. Microcytosis with a normal or raised RBC count and only a mildly low haemoglobin is the usual picture in thalassaemia trait, which is common across India, and the CBC alone cannot separate the two.
