How a Blood Group & Rh report is read
- Blood grouping is performed by both forward (cell) and reverse (serum) grouping.
- Rh (D) typing determines Rh-positive or Rh-negative status; weak-D testing may be required in selected cases.
- Grouping should always be confirmed on a fresh sample before any transfusion.
What commonly goes wrong with a Blood Group & Rh
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Forward (cell) grouping on its own is an incomplete group. Forward and reverse (serum) grouping must agree, and any discrepancy is investigated and resolved before a group is issued, never reported as-is.
- Reverse grouping is unreliable in infants below about six months, because their own ABO antibodies are still developing and maternal antibody may still be present. Their group is issued on forward grouping alone and the report must state that.
- Rh negative on initial typing is provisional in a donor until weak D has been excluded, because a weak D unit issued as Rh negative can immunise the recipient. A patient typed as Rh negative is managed as Rh negative, so the report must make clear which rule the lab applied.
- Most wrong blood groups are labelling errors, not technical ones. The tube is labelled at the bedside against the patient, and a group is never copied from an old card, a previous report or what the patient remembers.
