How a Thyroid Profile report is read

  • TSH exhibits diurnal variation and may vary with the time of sample collection.
  • Pregnancy and certain medications (e.g., biotin, corticosteroids, amiodarone, lithium) may influence thyroid hormone assay results.
  • Free T3 (FT3) and Free T4 (FT4) are preferred when alterations in thyroid-binding proteins are suspected.

What commonly goes wrong with a Thyroid Profile

The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.

  • TSH follows a daily rhythm, running highest in the late night and early morning hours and lowest in the afternoon and early evening. A borderline TSH should be repeated at a similar time of day rather than compared against a sample drawn at a different hour.
  • Biotin, present in many hair, skin and nail supplements, interferes with many immunoassay platforms and typically produces a falsely low TSH with falsely high T3 and T4, a pattern that reads exactly like thyrotoxicosis. Ask about supplements at registration, and arrange the draw after a washout interval agreed with the requesting doctor, since the necessary gap depends on the platform and the dose.
  • Total T3 and total T4 move with binding proteins. Pregnancy and oestrogen containing pills raise TBG, so total T4 rises in a woman whose thyroid function is entirely normal, and free T4 read against trimester specific ranges is the right basis in pregnancy rather than the adult non pregnant range.
  • A profile drawn during acute illness or an inpatient admission often shows a low T3 with an unremarkable or low TSH. That pattern belongs to the illness rather than the thyroid, and repeating after recovery is the standard approach.
