How a Testosterone, Total report is read

  • Testosterone concentrations exhibit diurnal variation and are highest in the early morning. Morning sample collection (typically between 7:00 AM and 10:00 AM) is recommended for adult males.
  • Total testosterone should be interpreted using age- and sex-specific reference ranges. Further evaluation with free testosterone and sex hormone-binding globulin (SHBG) may be indicated in selected clinical situations.
  • Testosterone concentrations may be influenced by obesity, acute illness, medications, and androgen replacement or suppression therapy.

What commonly goes wrong with a Testosterone, Total

The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.

  • Testosterone follows a daily rhythm and is highest in the morning, so the sample is taken between about 7 and 10 am. An afternoon sample runs lower, and a low result should be confirmed on a second morning sample before it is treated as a real finding.
  • Total testosterone tracks SHBG, and the total can mislead in either direction when SHBG is abnormal. Obesity, type 2 diabetes and hypothyroidism lower SHBG so the total reads low while the free fraction is normal, whereas ageing, oestrogen and liver disease raise SHBG and push the total the other way, and SHBG or free testosterone is what resolves the discrepancy.
  • Female and paediatric concentrations sit at the very bottom of the range of routine immunoassays, which is where those assays are least accurate. Low level samples need a method validated for that range, and results must be reported against sex and age specific reference intervals, never the adult male range.
  • Acute illness, recent surgery and significant stress transiently depress testosterone. A sample drawn from an unwell patient is not a basis for calling deficiency.
