How a Serum PSA (Total) report is read
- Serum PSA is organ-specific but not cancer-specific. Elevated PSA levels may be seen in benign prostatic hyperplasia (BPH), prostatitis, urinary tract infection, and following recent urological procedures.
- PSA results should be interpreted in conjunction with the patient's age, clinical findings, and digital rectal examination (DRE).
- Serial PSA measurements may provide additional clinical information. A single elevated PSA result is not diagnostic of prostate cancer.
What commonly goes wrong with a Serum PSA (Total)
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Draw the sample before the prostate is handled, digital rectal examination included. Prostate biopsy, catheterisation and cystoscopy can all raise PSA substantially, and about six weeks should pass after a biopsy before a value means anything.
- Ejaculation in the 48 hours before the test, an untreated urinary infection, prostatitis or acute retention all push PSA up. A single raised value in any of those settings is repeated once the cause has settled rather than accepted as the patient's baseline.
- Finasteride and dutasteride roughly halve PSA after about six to twelve months of use. A report that does not capture this medication makes a number look reassuring when it is not.
- PSA is prostate specific, not cancer specific. A raised value is most often benign enlargement, a value inside the reference range does not exclude cancer, and serial values must be compared on the same assay because methods differ between labs.
