How a Prolactin (PRL) report is read
- Prolactin concentrations may be transiently elevated by stress, exercise, sleep, and nipple stimulation. Where clinically indicated, repeat testing under standardized conditions may be considered.
- Hyperprolactinaemia may occur in pituitary disorders, hypothyroidism, pregnancy, renal disease, and with certain medications (e.g., antipsychotics, antidepressants, and metoclopramide).
- Mildly elevated prolactin results should be interpreted with caution and correlated with the patient's clinical presentation.
What commonly goes wrong with a Prolactin (PRL)
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Prolactin rises with stress, sleep, exercise, nipple stimulation and chest wall examination, so a difficult or repeated venepuncture can itself lift the result. Draw after the patient has been awake for a few hours and has sat quietly for a short rest period, and repeat a mildly raised value before it is treated as real.
- Macroprolactin, a large biologically inactive complex, is a well recognised cause of a raised prolactin in a patient with no symptoms. Where the number does not fit the clinical picture, PEG precipitation to check for macroprolactin comes before anything else.
- Very high prolactin can read falsely normal or only mildly raised on a one step immunoassay because of the hook effect. A strongly suggestive clinical picture with an unremarkable prolactin should be rerun on a diluted sample.
- Several very commonly prescribed drugs raise prolactin, including domperidone, metoclopramide and antipsychotics. Current medication should be recorded on the request form, because without it the result gets over interpreted.
