What commonly goes wrong with a Serum Creatinine
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Jaffe (alkaline picrate) methods are not specific. Bilirubin, haemolysis and high ketone levels all interfere, so a deeply jaundiced or ketotic sample needs confirmation by an enzymatic method rather than a straight report.
- Creatinine tracks muscle mass, so the same value means different things in a young muscular man and a thin elderly woman. This is why age and sex must be captured at registration and an eGFR reported alongside.
- A large meat meal transiently raises creatinine. A borderline value taken after a heavy non-vegetarian meal is worth repeating on a fasting sample before it is acted on.
- Units and the reference range must match. mg/dL and micromol/L differ by a factor of about 88, and a template carrying the wrong unit makes an ordinary result look alarming.
