How a Serum Calcium report is read
- Total calcium should be interpreted alongside serum albumin, as a low albumin lowers total (but not ionised) calcium; a corrected calcium may be calculated.
- Calcium is regulated by parathyroid hormone and vitamin D; abnormal values warrant their assessment.
- Prolonged tourniquet application can falsely raise the result.
What commonly goes wrong with a Serum Calcium
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Total calcium must be read against albumin. Roughly 40 to 45 per cent of circulating calcium is bound to protein, mostly albumin, so a low albumin lowers total calcium without true hypocalcaemia; the usual correction adds about 0.8 mg/dL for every 1 g/dL albumin falls below 4 g/dL.
- Any EDTA carry-over destroys the result. Filling the purple top before the plain tube, or topping up a plain tube from an EDTA tube, chelates the calcium and gives an implausibly low calcium together with a high potassium.
- A tight or prolonged tourniquet raises calcium by haemoconcentration, which is enough to move a borderline value. Release the tourniquet as soon as blood flows.
- Ionised calcium is a separate test with separate handling: anaerobic collection, no delay before analysis, and a result that shifts with sample pH. It is not interchangeable with corrected total calcium.
