How a Semen Analysis report is read
- Semen analysis should be interpreted in conjunction with the clinical history and, where indicated, a repeat sample after 2 to 3 days of sexual abstinence.
- Results are affected by the abstinence period, collection technique, completeness of the sample and the time and temperature of transport to the laboratory.
- Reference values vary with the criteria and method used by the laboratory.
What commonly goes wrong with a Semen Analysis
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Abstinence of 2 to 7 days is part of the test, not a suggestion. A shorter gap lowers volume and count while a longer one tends to reduce motility and vitality, and a report that does not record the abstinence period cannot be properly interpreted.
- The whole ejaculate must go into a wide-mouth sterile container. The first fraction carries most of the sperm, so a partly spilt sample under-counts badly, and an ordinary condom cannot be used because the lubricant is spermicidal.
- The sample must reach the laboratory within an hour of production, kept between 20 and 37 degrees Celsius and never refrigerated. Motility is read only after liquefaction is complete, usually within 15 to 60 minutes, and a count done on an unliquefied sample is unreliable.
- A single abnormal report is not a conclusion. Counts and motility vary widely between ejaculates from the same man, so an abnormal result is confirmed on a repeat sample collected some weeks later.
