How a Mantoux Test (TB) report is read
- The Mantoux test is read at 48 to 72 hours by measuring the transverse diameter of induration (not erythema) in millimetres.
- Interpretation of a positive result depends on the individual's risk category and BCG history.
- A negative test does not exclude tuberculosis, particularly in immunocompromised or recently infected individuals.
What commonly goes wrong with a Mantoux Test (TB)
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- The reading is the transverse diameter of the palpable induration, measured in millimetres. Measuring the surrounding redness is the single most common error and it over-calls positives.
- Read between 48 and 72 hours. A test that was not read inside that window must be repeated rather than estimated backwards from a later look.
- The injection must be intradermal and must raise a visible pale wheal at the site. If no wheal forms the dose has gone subcutaneous, the test is invalid, and it should be repeated at a site several centimetres away rather than read.
- BCG vaccination is near universal in India and can itself produce modest induration, so a history of BCG does not let you dismiss a large reading. Equally, a negative Mantoux does not exclude tuberculosis, since immunosuppression, malnutrition, HIV and severe disseminated disease all suppress the response.
