How a Dengue Profile report is read
- Dengue test results should be interpreted in conjunction with the patient's clinical presentation, duration of illness, and other laboratory investigations.
- NS1 antigen is generally detectable during the early phase of infection, while IgM and IgG antibody responses depend on the stage of illness.
- A negative result does not exclude dengue infection. Repeat testing may be considered if clinically indicated.
What commonly goes wrong with a Dengue Profile
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- Which dengue marker is useful depends entirely on the day of fever, so the requisition must record the day of illness. NS1 is the early marker and IgM appears only several days into the illness, which is why a day 2 IgM negative means nothing.
- A negative NS1 late in the illness does not exclude dengue, and NS1 is less sensitive in secondary infections. Late presentation is an IgM question, not an NS1 question.
- In an endemic population a positive IgG on its own reflects past exposure, not current dengue. IgG is only interpretable alongside IgM and the clinical timeline.
- Dengue IgM cross reacts with other flaviviruses that circulate in India, so a lone reactive IgM on a rapid card is a screening result and not a confirmed diagnosis. Serology also says nothing about severity, which is a clinical and haematological assessment, not a serological one.
