How a Lipid Profile report is read

  • Lipid profile results should be interpreted in conjunction with the patient's cardiovascular risk factors and clinical findings.
  • Non-fasting samples may influence triglyceride concentrations. Where applicable, fasting status should be considered during interpretation.
  • Calculated LDL cholesterol may be unreliable when triglyceride concentrations exceed 400 mg/dL.

What commonly goes wrong with a Lipid Profile

The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.

  • The fast matters mainly for triglycerides. 9 to 12 hours, water only, and no alcohol the previous evening; total and HDL cholesterol move little with food, triglycerides move a great deal.
  • LDL is calculated on most panels, not measured. The Friedewald calculation is not valid when triglycerides exceed 400 mg/dL, and a report that prints an LDL anyway is publishing a number that does not exist.
  • Cholesterol falls transiently after a heart attack, acute illness, surgery or major infection, so a panel drawn in that window understates the patient's usual level.
  • A prolonged tourniquet raises every measured lipid by haemoconcentration, which is enough to move a borderline result across a decision line.
