- A raised homocysteine level is a recognised risk marker for cardiovascular and thromboembolic disease.
- Results are affected by sample handling; the plasma should be separated promptly to avoid falsely high values.
What commonly goes wrong with a Homocysteine
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- This is one of the most handling-sensitive tests on a routine menu. Red cells keep releasing homocysteine into plasma after collection, so the tube must go on ice and be centrifuged promptly; a sample left at room temperature reads falsely high.
- Collect fasting. A methionine-rich meal raises the level, so a post-meal sample is not comparable with a previous report.
- A raised homocysteine is not specific. B12 or folate deficiency and impaired renal function raise it too, which is why it is read alongside those results rather than on its own.
