How a HbA1c report is read
- HbA1c reflects the average blood glucose concentration over the preceding 2 to 3 months and is not affected by recent meals.
- HbA1c results may be unreliable in conditions affecting red blood cell survival (e.g., haemolytic anaemia, recent blood transfusion, haemoglobin variants, significant blood loss, or pregnancy).
- Results should be interpreted in conjunction with clinical findings and plasma glucose measurements where appropriate.
What commonly goes wrong with a HbA1c
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- HbA1c is not a fasting test and needs no diet preparation. Sending a patient home to come back fasting for it wastes a visit and changes nothing.
- It reflects average glycaemia over roughly the previous two to three months, weighted towards the most recent weeks. It will not show hypoglycaemic episodes or day to day swings, and one careful week before the test does not move it.
- Anything that shortens or alters red cell lifespan invalidates it, including recent transfusion, haemolysis, recent blood loss, pregnancy and chronic kidney disease. Untreated iron deficiency anaemia, which is very common in India, is reported to shift HbA1c independently of glucose, so an anaemic patient's HbA1c should be interpreted with the haemogram in hand.
- Haemoglobin variants and thalassaemia trait can interfere depending on the method, so the report should state the method and, on HPLC, the chromatogram should be looked at before the number is released. Keep the units consistent as well, since % and mmol/mol are two different scales for the same result.
