How a Blood Sugar (FBS & PLBS) report is read

  • Blood glucose results should be interpreted in conjunction with the clinical findings and, where indicated, HbA1c and a repeat measurement.
  • A fasting sample requires 8 to 10 hours of fasting; the post lunch sample is collected 2 hours after a meal.
  • Values may be affected by acute illness, stress and certain medications.

What commonly goes wrong with a Blood Sugar (FBS & PLBS)

The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.

  • Fasting means 8 to 12 hours with water only. Tea with sugar, a biscuit, or a fast stretched well past 14 hours all make the value unreliable, and "I only had tea" is the single most common broken fast in Indian OPD collection.
  • Post prandial is timed two hours from the first mouthful of the meal, not from when the patient finishes eating and not from when they reach the lab counter. A patient who eats a deliberately light meal, or skips it altogether after giving the fasting sample, produces a PP figure that means nothing.
  • Cells in the tube keep consuming glucose after collection, so an unseparated sample left on the bench reads falsely low. Use a fluoride tube and still separate promptly, because fluoride takes a couple of hours to fully arrest glycolysis, and a plain clot tube standing for hours in the heat is a silent source of low results.
  • A glucometer capillary reading and a laboratory venous plasma glucose are not interchangeable. Glucometers are built for monitoring, and a diagnosis should not rest on one.
