How a Arthritis Profile report is read
- Rheumatoid factor is not specific for rheumatoid arthritis and may occur in other autoimmune disorders, chronic infection and some healthy individuals; Anti-CCP is more specific and supports the diagnosis.
- A negative result does not exclude rheumatoid arthritis, particularly in early disease.
- ASO titre reflects recent streptococcal infection and CRP reflects active inflammation; results should be interpreted together with the clinical findings.
What commonly goes wrong with a Arthritis Profile
The practical errors that actually cause repeat samples and misread results, mostly at collection and at the point the result is read.
- RA factor neither confirms nor excludes rheumatoid arthritis: it is negative in a substantial minority of genuine cases and positive in healthy elderly people and in chronic infections such as tuberculosis, leprosy and hepatitis C. A positive latex screen should be quantified by titre through serial dilutions rather than issued as a bare positive.
- ASO reflects past streptococcal exposure, not current infection, and one raised titre in a population where exposure is common carries very little weight on its own. Paired samples showing a rising titre are what matter, and the ASO response is poor after streptococcal skin infection, so a normal value does not exclude recent strep.
- Uric acid is not a gout test. It is often normal during an acute attack and raised in plenty of people with no joint symptoms at all, and the reference range is sex specific, which labs applying a single range to everyone get wrong.
- CRP tells you inflammation is present, not where or why. It rises with any infection or tissue injury, a normal CRP does not exclude inflammatory joint disease, and high sensitivity CRP is a separate assay with its own reporting range that should not be swapped in for the routine one.
